Bereavement Services
At Transitions Hospice, we understand the difficulty associated with losing someone you love, and we want to help ensure that loved ones are left with positive memories and peace of mind.
To provide relief to surviving family members and friends, we offer ongoing bereavement services and follow-up to help answer their questions, manage their grief, and support them both spiritually and emotionally.
Our Bereavement Services Include:
- Up to 13 months of bereavement support after death
- Bereavement literature mailed to homes
- Grief and loss support visits by the Spiritual Care Coordinator and/or Social Worker, as needed
- An annual memorial service for family and friends.
